WHAT IS SOCIAL PRESCRIBING?

White heart shape on a black background.

Social prescribing is a way to support young people’s health and wellbeing by connecting them with activities, groups, and services in their community. It’s about finding the right support for each young person, whether that’s joining a local club, trying new hobbies, getting active, or accessing specialist services so they feel happier, more connected, and supported in their everyday life.

Our Social Prescribing Link Workers work across Plymouth, helping young people explore activities and services that build confidence, connections, and wellbeing.

SCHOOL
SUPPORT

We also offer in school support for students who have experienced trauma or have special education needs. We focus on promoting active lifestyles and supporting emotional wellbeing through physical activity.

Our approach

  • Cartoon illustration of a black cat face with big eyes and a pink nose, with a pink arrow pointing left.

    YOU SET THE GOALS

We focus on what matters to you or your family, not what’s decided by services.

  • Stacked colorful blocks with increasing height and a green block in the middle.

    CONNECTED SUPPORT

We work with local groups, schools, and networks to make sure you get the right help in the right place.

    ALWAYS LEARNING

We use feedback and experiences to make our support even better for young people across Plymouth.
