WHAT IS LIFT?

LIFT is a youth work-led service supporting young people who have been admitted to hospital, or who are at risk of hospital admission.

We'll get to know what's important to you, at your pace, by listening, taking part in activities you choose, and exploring what you'd like help with. Joining LIFT is your choice, and if you decide to take part, we'll work together to build a plan around the goals that matter most to you.

Your LIFT youth worker will take time to get to know what matters to you, at your pace, through listening, activities you enjoy, and conversations about what you’d like help with.

Support can take place face-to-face, online, or over the phone.

WHO IS THIS SERVICE FOR?

We work with young people who have been referred to us by a hospital ward or a Mental Health Crisis Team.

This referral pathway helps us ensure that the service reaches the young people it is designed and funded to support, so they receive the right support at the right time

If you decide to take part, we'll ask you to:

  • Meet with your youth worker

  • Be yourself and help us get to know you

  • Work together to create goals that are important to you

  • Try new things, when you're ready

  • Celebrate your progress along the way

OUR SUPPORT IS

EFFECTIVE

There are lots of ways we can help you; setting goals, advising on options, working with you on issues, and meeting you regularly. 

CONFIDENTIAL

Our team of youth workers will keep what you tell them confidential (between the two of you) unless they have concerns about the safety of yourself or others. All LIFT youth workers are supervised and managed, to make sure they‘re working safely and professionally.

FREE AND ACCESSIBLE

Your youth worker can meet you on the ward, where you live, or around your local area. They can work with you for up to 6 months.
