WHAT IS IN REACH?

In-Reach is for young people aged 11–17 who are registered with a Devon Doctors Surgery (including Torbay and Plymouth) and have been admitted, or are at risk of being admitted, to hospital.

YOUTH WORK

You and your youth worker will work together, to build a relationship and get to know one another. Your youth worker will give you the chance to talk face-to-face, listen to what’s important to you, and do the things you like to do.

FINANCIAL SUPPORT

Help can be available to support your plan and help you achieve your goals. Your youth worker will work with you, to ensure your goals are sustainable long-term. 

PERSONAL PLAN

You and your youth worker will work together to make a plan around what you need help with, and what you want to change in your life. Your plan can be made any way you want e.g. with drawings, or voice recordings.

There are lots of ways we can help you; setting goals, advising on options, working with you on issues, and meeting you regularly. 

CONFIDENTIAL

Our team of youth workers will keep what you tell them private, unless they have concerns about the safety of yourself or others. All In-Reach youth workers are supervised and managed, to make sure they‘re working safely and professionally.  

FREE AND ACCESSIBLE

Your youth worker can meet you on the ward, where you live, or around your local area. They can work with you for up to 7 months.    


Please be advised that referrals to the Young Devon In-Reach service can only be made through specific channels. To ensure proper coordination and support, all referrals must come directly from either the hospital ward or your CAMHS Crisis worker.
