Transitions project
Historically the passage for a young person who requires support, due to their identified special needs, from Children to Adult services, has been clouded by uncertain responsibilities, different interpretations of legal duties and differences in available resources. Parent Carers have consistently cited this passage as a most stressful and traumatic time. Rather than a celebration of a child reaching legal adulthood, it often ends in confusion, added hardship and pitching professionals against each other, which often deflects the focus away from presenting needs.
As a result of this Young Devon has been asked to:
- Lead the development of the Children’s & Young People’s Transition Implementation Group
- Identify options and support the implementation for CYP and Adult services directorates regarding the delivery of an integrated Transition planning and support function in Devon.
- Identify Protocols and Processes for Transition between the ages of 14-25
- Present options to services for activity between the ages of 16-21.
Aim
To ensure that the transition of children & young people (aged 14 – 25) with additional needs from children’s (Social Care, Health & Education) services to Adults services is a supportive & positive experience.
Objectives:
- To work alongside the Children’s & Young People’s Transition Implementation Group in achieving the outputs identified within the plan
- To identify all children & young people (aged 14 – 25) with additional needs so as to better understand how the interface between children’s (Social Care, Health & Education) services to Adults services currently works and can be improved.
- To examine the range of systems currently used by all agencies with a current or future responsibility towards this group, with a view to developing a better understandin
- To talk with and listen to children & young people (aged 14 – 25) with additional needs and their parents/Carers when looking for improvements.
- To test out Protocols and Processes for Transition between the ages of 14-25
- To present options for further activity to the relevant decision making groups.
